A dilated feeder artery, draining vein, pre-retinal fibrosis, and tractional retinal detachment can be seen. Two small retinal capillary hemangioblastomas and subretinal fluid involving the macula were also associated with the hemangioblastoma. Systemic and genetic work-up revealed a diagnosis of Von Hippel-Lindau disease.

The patient reported a history of reduced vision in both eyes, first noticed at 4 years of age. Fundus images showed bilateral posterior staphyloma with retinal pigment epithelium atrophy at the macula, and pigmented scar at macula and periphery.
